This Free Special Education IEP Parent Input Survey helps teachers gather meaningful information from parents and guardians before the IEP meeting so the team can create a more personalized and effective plan for the student.

This informal parent questionnaire is designed to encourage family participation and ensure that parent voice is included during the IEP development process.
